What Psoriasis Actually Is

In healthy skin, keratinocytes migrate from the basal layer to the surface over roughly 28 days. In a psoriatic plaque, that transit takes 3 to 5 days. The result is the thick, silvery scale patients recognize — skin cells arriving at the surface before they have matured or shed properly.

The acceleration is driven by immune signaling, not by the skin itself. The best-characterized pathway is the IL-23/Th17 axis:

  • Dendritic cells in the skin, activated by stress signals, infection, or injury, release IL-23.
  • IL-23 drives differentiation and survival of Th17 lymphocytes.
  • Th17 cells secrete IL-17A, IL-17F, and IL-22.

Common Triggers

  • Physical trauma to skin — the Koebner phenomenon, where plaques appear at sites of injury, scratching, or even tattoo lines

Psoriatic Arthritis: The Part Patients Miss

Roughly 20 to 30 percent of people with psoriasis develop psoriatic arthritis (PsA). In the large majority, skin disease comes first — often by around a decade — which means a psoriasis diagnosis is an opportunity to screen for joint disease before damage occurs. Erosive joint change in PsA can begin within the first two years of symptoms, and it is not reversible.

Watch for:

  • Morning stiffness lasting longer than 30 minutes, improving with movement (inflammatory, not mechanical)
  • Dactylitis — a whole finger or toe swollen uniformly, the classic "sausage digit"
  • Enthesitis — pain at the Achilles insertion, plantar fascia origin, or lateral elbow that does not behave like ordinary tendinopathy
  • Nail changes — pitting, onycholysis, oil-drop discoloration; nail involvement substantially raises PsA risk because the nail bed is anatomically continuous with the distal interphalangeal enthesis
  • Inflammatory back pain — night pain, morning stiffness, better with activity, worse with rest
  • Asymmetric joint involvement, often distal interphalangeal joints, unlike rheumatoid arthritis

Disease characterization

  • Nail assessment

Inflammatory markers

  • hs-CRP and ESR — imperfect in psoriasis but useful for trend
  • CBC with differential

1. Do Not Abandon Effective Conventional Therapy

For moderate-to-severe psoriasis, biologics targeting IL-17, IL-23, or TNF are the most effective tools available, with many patients achieving near-complete clearance. Integrative medicine done responsibly does not ask patients to refuse them. For mild disease, topical vitamin D analogues, topical steroids used correctly and intermittently, and phototherapy remain reasonable and evidence-supported first-line options.

2. Nutrition Aimed at the Inflammatory Substrate

The dietary evidence in psoriasis is stronger than skeptics assume, though it is about the pattern rather than any single food:

  • Mediterranean-pattern eating is associated with lower psoriasis severity in observational cohorts, likely via polyphenols, monounsaturated fat, and fiber-driven short-chain fatty acid production.

  • Alcohol reduction matters, both as an independent severity driver and because of interaction with systemic medications.
  • Gluten elimination helps a defined subgroup: patients with positive celiac or gliadin serology. It is not a universal recommendation, and testing before eliminating is the correct order of operations.

We do not put patients on elaborate restrictive protocols without a reason. The goal is a sustainable anti-inflammatory pattern plus targeted correction of what testing actually shows.

3. Correcting Metabolic and Nutrient Drivers

Insulin resistance amplifies inflammatory signaling, and psoriasis patients have higher rates of it even at normal weight. Addressing it — through resistance training, protein-adequate nutrition, sleep, and where clinically appropriate pharmacologic support — is genuinely disease-modifying rather than cosmetic. Patients ask about peptides constantly, so here is the accurate picture. There are no large randomized trials of peptide therapy for psoriasis. What exists is mechanistic and early-phase data on immune-modulating and tissue-repair peptides, and clinical experience in adjacent inflammatory conditions. Anyone promising that peptides will clear moderate-to-severe psoriasis is overselling. We will tell you what is established, what is plausible, and what is unproven.

6. Regenerative Options for Damaged Joints

When PsA has been controlled systemically but a specific joint or enthesis remains symptomatic from structural damage, regenerative approaches may have a role — after, not instead of, immune control. Frequently Asked Questions

Is psoriasis contagious? No. It is an immune-mediated process, not an infection of the skin surface.

Can psoriasis be cured? No current therapy cures it. Remission and near-complete clearance are realistic goals for many patients, and comorbidity risk can be meaningfully reduced.

Should I stop my biologic if my skin clears? Not without your prescriber's input. Discontinuation frequently leads to relapse, and in some cases reduced response on restarting. Deprescribing, if appropriate, is a planned decision.
