Stage 1: Freezing (roughly 6 weeks to 9 months)
Pain dominates. It is often worse at night, poorly localized, and may radiate into the deltoid insertion. Range of motion is decreasing but the loss may still feel like guarding rather than a hard stop. This is the inflammatory phase, and it is the window in which anti-inflammatory intervention has the greatest effect on the total duration of the disease.
Stage 2: Frozen (roughly 4 to 12 months)
Pain begins to settle, but stiffness is now the defining problem. The hallmark exam finding is loss of passive external rotation with the arm at the side — meaning the shoulder will not rotate even when someone else moves it for you.
