What Is Estrogen Dominance?

Estrogen stimulates cell proliferation, fluid retention, and the uterine lining; progesterone opposes these effects. When this balance tips toward estrogen—regardless of whether absolute estrogen levels are "normal"—the clinical picture of estrogen dominance emerges.

Three Patterns of Estrogen Dominance

1. High Estrogen, Low Progesterone The most classic pattern: estrogen is elevated (due to external sources, impaired liver clearance, or adipose aromatization) while progesterone is deficient. Common in PCOS, perimenopause, and women with impaired progesterone production.

2. Normal Estrogen, Low Progesterone Estrogen is within reference range, but progesterone has declined such that estrogen's effects are no longer balanced. Common in luteal phase deficiency and early perimenopause when ovulation becomes irregular.

3. Impaired Estrogen Metabolism Estrogen is produced normally but metabolizes through unfavorable pathways—elevated 16-OH estrone relative to 2-OH estrone. This creates local tissue exposure to more potent estrogen metabolites even with normal serum estradiol.

Root Causes of Estrogen Dominance

Impaired Hepatic Estrogen Clearance

The liver converts active estrogens to inactive metabolites for excretion. Impaired liver function—from fatty liver, alcohol use, nutritional deficiencies, or high toxin burden—slows estrogen clearance and allows recirculation.

Gut Dysbiosis and the Estrobolome
