Standard treatment is levothyroxine (T4) — a synthetic thyroid hormone precursor that the body is supposed to convert into active T3. However, a significant subset of patients have impaired T4-to-T3 conversion due to:

  • Nutritional deficiencies (selenium, zinc, iodine)
  • Chronic stress and elevated cortisol
  • Gut microbiome dysbiosis
  • Genetic polymorphisms in the DIO2 enzyme responsible for T4 conversion

These patients take their levothyroxine faithfully, have "normal" TSH, and still experience fatigue, weight gain, brain fog, depression, hair loss, and constipation — because they are not converting T4 to the T3 their cells need.
